The Upstate Homeless Coalition of South Carolina operates TRANSITIONS 2000,
a transitional housing program, in six
upstate counties. The program provides housing, intensive case management,
and other supportive services to help participants work towards
self-sufficiency. Program participants may remain in transitional housing
up to two years, so long as progress towards their goals is being made.
While not everyone will become self-sufficient, our philosophy is to offer
the opportunity to qualified applicants. Our key to success is an
intensive case management approach, which emphasizes daily contact, weekly
meetings, and resident accountability. Assessment for acceptance into
the program is done by the case manager.
Some of our funding partners for Transitions 2000 are:
the U.S. Department of Housing and Urban Development,
S.C. Office of Economic Opportunity,
the Spartanburg County Community and Economic Development,
Greenville County Redevelopment Authority,
and the
United Way of the Piedmont.
The Ronald McDonald House Charities and ScanSource
contribute money to help homeless children in the program with extracurricular activities.
Private donations are not only welcome, but are very much needed to continue offering a
holistic program that takes the homeless families/individuals where they are and moves
them towards self-sufficiency.
Application Process
To be considered for TRANSITIONS 2000, potential participants must be
assessed by the
case managers.
The person must be homeless by HUD definition, must be willing and able to
work towards self-sufficiency, and must not have used illegal substance or
alcohol in the last 90 days. HUD income guidelines are used. The
assessment process consists of a minimum of three meetings with a case
manager. Individual goals are discussed during the assessment.
Referrals can be made by other agencies or potential participants can self
refer. A referral form can be obtained from the case managers, or you
can mail or fax
