Transitions 2000 -
A transitional housing program, in six
upstate counties. The program provides housing, intensive case management,
and other supportive services to help participants work towards
self-sufficiency.
Continuum Care -
The Continuum of Care (COC) is the term used for the process of coordinating services and
planning among all the homeless service providers in the area.

The HOME program is permanent housing for people with mental illness.

Ten Year Plan -
The Federal mandate is for the nation's communities to work and implement a
ten year plan to eliminate homelessness. To this end the S.C. Homeless
Coalition has written a state plan that has been distributed to the various
communities to adapt to their own needs.
