Transitions 2000 - This program provides housing, intensive case management, and other supportive services.

Continuum Care - The Continuum of Care (COC) is the term used for the process of coordinating services and planning among all the homeless service providers in the area.

Homeless Person - If you are homeless or may become homeless in the near future then you may qualify for one of our programs.

Home/Shelter Plus Care - The HOME program is permanent housing for people with mental illness.  UHCSC provides housing and case management.

Ten Year Plan - The Federal mandate is for the nation's communities to work and implement a ten year plan to eliminate homelessness.
